Treatment Phases

There are two main phases of treatment at PATH’s main residence, with the additional option of Aftercare Wraparound in the nearby community:


Phase I

Phase I lasts typically 6-12 weeks, but may take longer depending on the needs of the Resident. This phase is focused on the evaluation and transition of the Resident to PATH.

During this time, Residents move into the main house and begin working intensively with their Primary Clinician (3x/week) to establish a treatment relationship and gain better understanding of the areas to focus on in individual sessions. They will also identify and access additional resources as needed to support therapeutic work, including prescribed groups, weekly treatment team meetings, psychiatric evaluation and medication monitoring, family/parent coaching, CBT/DBT therapy and vocational support. Additionally, Residents can choose to participate in recreational/social activities offered both on and off campus.

When Residents feel prepared for their next step, they work with their Primary Clinicians to move to Phase II.


Phase II

Phase II lasts typically 6-12 weeks, but may vary depending on the needs of the Resident. In Phase II, residents continue living in the main house and participating in treatment activities as established in Phase I, or as determined in collaboration with the Resident and their treatment team. Additionally, Residents work intensively with the Vocational Director to learn about and apply for opportunities in volunteering, working, or attending classes (online or in person) and are encouraged and assisted in beginning to engage more with the outside community.


Aftercare Wrap-Around

As Residents become more comfortable in applying learned skills and extending their capacities to a work and/or school environment, they may choose to become an Aftercare Wrap-Around Program client. This Program is designed as a transitional living program which typically lasts 8-16 weeks but may vary depending on the needs of the Client.

When in the Aftercare Wrap-Around program, Clients typically move out of the main house and into a PATH-sponsored/managed apartment located in a neighboring town. Aftercare Wrap-Around Clients are able to continue receiving therapeutic, vocational, and medication management support through PATH - both at the main residence as well as in the community - in order to support the Client’s efforts to live more independently. Clients will have an established plan for living, including individual and/or group therapy, community and/or online-based support groups (such as AA, NA, etc.), and active employment, volunteer work, and/or educational pursuits. Additionally, the plan will include a personalized relapse prevention plan to encompass psychiatric and substance use disorder challenges.

Clients can also choose to receive case management and care coordination services, life skills coaching (e.g., budgeting, cooking, time management, cleaning, pursuing interests/managing free time, etc.), therapeutic support, and referral assistance from the Primary Clinician. The Primary Clinician works with the Client and treatment team to outline aftercare goals and track progress in order to inform the next steps for the Client.

Aftercare

When meaningful progress has been made on a Client's aftercare goals (as determined by the Client and their treatment team) the Client transitions from the Aftercare Wrap-Around Program to PATH aftercare. Clients who are ready to go into full aftercare are supported by the Primary Clinician and members of the treatment team in their transition to their next step, be it moving into their own apartment or other independent living arrangement, returning to their home community, attending school, or entering another program. When Clients enter this stage, they continue to be considered part of the PATH community and are welcome to attend PATH activities as guests.

Alternatively, if a Client decides to move from Phase II back to their home community rather than to PATH-based Wrap-Around services, members of the PATH treatment team will work collaboratively with the Client and, if agreed, their family to identify treatment providers and appropriate services in the client's area and provide transitional support as needed.
